Behavioral Screens

At Wilmington Pediatrics we are dedicated to providing high quality, comprehensive health care to our patients.

Each time a child comes in for a well child visit, we ask that the parents/guardians complete a social/emotional screen.  This tool helps us to identify potential areas of risk for both the child and the family.

Dr. Ellen Perrin, a developmental pediatrician at Tufts Floating Hospital for Children has created a screen for children ages 5 and under.  The screen is called the SWYC-survey of well being for young children. This is for children ages 0 thru 5 years.

Some of the information on the screening is related to the family and for your privacy, we are asking you to complete the screen on our patient portal. 

All information provided through the portal is secure and will be reviewed by the appropriate clinical staff.  To access the portal click on the link on the portal page.

For children ages 6 - 18, you can complete the appropriate screening below and submit it to our office or you can complete those screens on the portal as well.

We do have copies of the screening tools in our office for anyone who wishes to complete the screen while they are in the office.  Please be advised that the screen needs to be completed and given back to the secretaries before the clinical staff can take your child in to an exam room.
