Well Child Exam Visit Handouts and Developmental Screens

The pediatricians at Wilmington Pediatrics follow the guidelines set for by the American Academy of Pediatrics.  Below are the links to the well child exam handouts created by AAP and followed by Wilmington Pediatrics.  The information contained in the handouts will be discussed at your child’s appointment.

For your convenience, underneath each Well Child Exam is a link to the appropriate developmental screen that we ask you to fill out before your child’s appointment.  You can also fill out these screens on the patient portal.

Patient portal screens will be submitted and are recorded in your child’s progress note.  For developmental screens completed on the website, you will need to print out the form and bring it with you on the day of your appointment.
